Permission for Young Person to Volunteer

I give permission for the young person (details below) to take part in the volunteering role set out in the covering email, including all the activities involved.

I have understood the requirements of this role and my permission covers all activities involved.

I understand that staff, other volunteers and organisation representatives will take reasonable care of all volunteers and they cannot be held responsible for any loss, damage or injury my young volunteer may suffer as a result of their volunteering activity.

10. I agree to staff giving my child’s emergency contact details from this form to the emergency services, so they can inform the emergency contact in order to gain permission for any treatment or medication considered necessary by the medical authorities present*

11. Please provide your full name as the parent or guardian of the young volunteer as acceptance of the answers above (this is as an alternative to a signature)*
